We are located between Farmington and Levan Roads, just east of Levan Road on the south side of Five Mile Road. St. Mary’s Hospital is just west of Levan Road and Five Mile Road. We are approximately one mile north of the I-96 Expressway. Our facility is a one-story brick building with our address visible from Five Mile Road.
